Ingleside Communities, a long-time provider of senior health and living services in the Mount Horeb community, is excited to announce a new business that is opening under their umbrella. Transitions At Home provides home health care and non-medical home care services that enrich and protect the lives of those who value independence. Their services help seniors and the disabled stay comfortably and safely at home. Knowledgeable and compassionate licensed nursing staff, therapists and home health aides, combined with today’s top technology and personalized care plans, bring an exceptional and convenient in-home experience. Their team of healthcare professionals strive to promote compassion, comfort, dignity, quality, and respect to patients and the community.

Owned by Wisconsin Illinois Senior House Inc., a non-profit holding company, Transitions at Home relies partially on donations from the community, which are tax-deductible and directly help provide improved and expanded services to their patients. It also allows them to help patients who may not be able to afford in-home care.
Serving a wide area, including Dane, Columbia, Green, Sauk, Iowa, Lafayette, Richland, and Grant Counties, Transitions at Home is set to join Ingleside as one of the community's central employers and providers. The Mount Horeb Area Chamber of Commerce is excited to welcome Transitions At Home to the chamber family. “We are so fortunate to have such quality care in own backyard,” says Rachel LaCasse-Ford, Director of Business Development at the Chamber. “This is just one more feature that makes Mount Horeb a great place to live and operate a business.”
Currently operating out of Ingleside, Transitions At Home will be moving to an office in the Mount Horeb Innovation Center on Main Street. They are looking forward to celebrating with a Ribbon Cutting this summer. Please contact Jacob Rathke to find out more, to inquire about career opportunities, and to offer financial support to this essential service they are bringing to Mount Horeb and the surrounding counties.
